Redundant and lax eyelid skin and muscle is known as dermatochalasis. Dermatochalasis is a common finding seen in elderly persons and occasionally in young adults. Natural aging: gravity, loss of elastic tissue in the skin, and weakening of the connective tissues of the eyelid frequently contribute to this lax and redundant eyelid tissue. These findings are more common in the upper eyelids but can be seen in the lower eyelids as well.

In some cases, severe stress or trauma can accelerate the aging process, resulting in dermatochalasis at an earlier than normal age. Some systemic diseases also may predispose patients to develop dermatochalasis. These include thyroid eye disease, renal failure, trauma, cutis laxa, Ehlers-Danlos syndrome, amyloidosis, hereditary angioneurotic edema, and xanthelasma. Genetic factors may play a role in some patients.

Dermatochalasis can be a functional or cosmetic problem for the patients. When functional, dermatochalasis frequently obstructs the superior visual field. In addition, patients may note ocular irritation, entropion of the upper eyelid, ectropion of the lower eyelid, blepharitis, and dermatitis. When cosmetic, patients note a fullness or heaviness of the upper eyelids, "bags" in the lower eyelids, and wrinkles in the lower eyelids and the lateral canthus.

Dermatochalasis can be separated into early and late phases. The early phase is divided further into hypertrophic and atrophic forms. The cause is probably a localized form of angioedema. Sequelae include conjunctival edema and injection, entropion, ectropion, steatoblepharon, ptosis, and excessively thin skin.

Blepharochalasis syndrome is separate and distinct from dermatochalasis and is a rare disorder that typically affects the upper eyelids. Blepharochalasis syndrome is characterized by intermittent eyelid edema, which frequently recurs. This results in relaxation of the eyelid tissue and resultant atrophy. In approximately 50% of patients, it is unilateral. Blepharochalasis rarely can be associated with agenesis of the kidney, vertebral abnormalities, and congenital heart defects.

TREATMENT

In general, the treatment of dermatochalasis is surgical. The following medical treatments may be appropriate:
  • Dermatochalasis patients with blepharitis may benefit from lid hygiene and topical antibiotics.
  • Dermatochalasis patients with dermatitis may benefit from topical steroid ointment.
  • Dermatochalasis patients with dry eyes should be treated with the appropriate topical lubricant. In addition, placement of temporary collagen punctal plugs, permanent punctal plugs, or punctal cautery may be considered in patients with a history of dry eye or a physical examination consistent with dry eye. These measures may be used preoperatively to further evaluate the patient prior to embarking upon surgery.

Chronic progressive external ophthalmoplegia (CPEO) is a condition where all the external eye muscles gradually become paralyzed. The cause is in the DNA. CPEO is a slowly progressing and rare disease that may affect those of all ages, but typically manifests in the young adult years. The first presenting symptom of ptosis is often unnoticed by the patient until the lids droop to the point of producing a visual field defect. Often, patients will tilt the head backwards to adjust for the ptosis of the lids. In addition, as the ptosis becomes more severe, the patients will use the frontalis (forehead) muscle to help elevate the lids. The ptosis is typically bilateral, but it may be unilateral for a period of months to years before the other lid becomes involved.

SYMPTOMS

First and most striking features are: the drooping upper eyelids of both eyes (with obstructed vision).

Other features include:
  • Impaired eye movements, making it increasingly difficult and necessary to move the head to see and follow. This progresses very slowly.

Blepharophimosis Ptosis Epicanthus inversus Syndrome is a congenital and rare syndrome, where it may be necessary at a very young age to surgically correct the ptosis for the development of good eye sight and avoid a lazy eye and strabismus. 
Blepharophimosis, ptosis, and epicanthus inversus syndrome, either with premature ovarian failure (BPES type I) or without (BPES type II), is caused by mutations in the FOXL2 gene. 

BPES eyelid abnormalities are:

  1. Blepharophimosis: Blepharo means eyelids and 'Phimosis' means closed opening. Blepharophimosis is a strong narrowing of the eyelids (narrowing between the inner and outer corner). In adults, the width of the lid 25 to 30 mm. In adults with BPES this distance is 20-22 mm
  2. Ptosis: both eyelids droop (symmetric) and can hardly be lifted;
  3. Epicanthus inversus: A condition in which a fold of skin that stretches from the lower eyelid upward and toward the nose partially covers the inner canthus. It is often associated with ptosis;
  4. Telecanthus: abnormally increased distance between the medial canthi of the eyelids. There is usually no visible sclera (white of the eye) between the corner and the iris.

Given the severity of the ptosis, the impact it may have of the development of vision and the eyes and the young age of patients with BPES, it is advisable to (only) rely on the advice given by a physician.

Michael Calderone's migraine headaches have been so crushing that life his life would just stop.

"I would come home in the summer after working and go pass out in a dark room with a headache so bad I would lose the rest of the day," said Calderone (52-year-old) . "I was totally dysfunctional until I passed out." The headaches only got worse as he grew older.
"If it hit in the morning, I would lose a whole day and the next day until it would cycle down," he said. "No bright lights, no stress, no activity at all -- just not to agitate your head so it would not get worse."
 That was until he met Dr. Bahman Guyuron, chair of the department of plastic and reconstructive surgery at University Hospitals Case Medical Center. Guyuron had pioneered a surgical technique that he had discovered by accident while doing cosmetic surgery on women.

In July 2008, Guyuron gave Calderone a forehead lift, freeing entrapped nerves behind his eyes that had caused chronic and excruciating pain on the left side of his face.
And today, the migraines are gone. "At one point I was getting them every day --- I was incapacitated," said Calderone. "Now, I feel like I have my life back."

According to the Migraine Research Foundation, an estimated 36 million Americans -- about 10 percent of the population, suffer from migraines. They are not just headaches, but an array of neurological symptoms that can include throbbing head pain, nausea and visual disturbances. Migraine ranks in the top 20 of the most disabling medical conditions.

Three times as many women as men are afflicted, according to the foundation, which funds research and provides medical resources for patients. Migraines tend to run in families. Both Calderone's 21-year-old daughter and his niece have them. Attacks can last between four and 72 hours and may include nausea, vomiting, as well as extreme sensitivity to sound, light, touch and smell. Tingling or numbness can also occur in the extremities or face.

But now, plastic surgeons, many of whom have been trained under Guyuron, are offering nerve decompression surgery. He theorizes that 90 percent of all migraines are caused by irritation of the peripheral branches of the trigeminal nerve. Guyuron first discovered the connection between migraine and nerve compression by muscles, vessels and bone 12 years ago."A surgeon's wife had a forehead lift and came for a follow-up with me," he said. "She told me she was not only happy with the way she looked, but she hadn't had a migraine for the previous six months."
"I thought it was a coincidence, but another patient told me the same thing," said Guyuron. So he investigated 314 patients who had undergone forehead lifts over the previous decade, revealing that 31 of 39 who reported migraine headaches saw "significant improvement." "That was the beginning," he said.

A Guyuron-led study published in the journal Plastic and Reconstructive Surgery in 2009, found that nearly 85 percent of patients who underwent the nerve decompression surgery had a 50 percent reduction in migraine.
His work suggests that migraine arises from irritation in the nerves, which releases toxins and begins a "cascade of events that end up being a full-blown migraine in the head." According to Guyuron, there are four common trigger points on the face and four less common ones. Most are on the forehead, temple, back of the head and behind the eyes."Each one has a different mechanism and reason for the nerves being irritated, and we have developed a surgical technique for each trigger," he said.

Plastic Surgeons Try Botox First

Surgery is done on an outpatient basis and lasts about 45 minutes to an hour for each trigger point that is deactivated. In three out of four cases, it is a muscle that is compressing the nerve, though it can be bone or even the cartilage of a deviated septum. Guyuron first tests a patient with Botox, which paralyzes the muscle, to see if it has a positive effect. "If they respond to Botox, I know they will respond to surgery well," he said. 

For Calderone, Botox injected above his left eyebrow and it had a remarkable effect. "It would stop it for three or four months, then the migraines would begin again," said Calderone. "But it was still better than getting them every day." But after three years, he decided to look to a more permanent treatment. 
Calderone paid $10,000 out of pocket for his surgery, but he estimates he was spending nearly as much on Botox, injections and medications.
Guyuron operated on the muscle across his nose. "In three and a half years, I haven't gotten a migraine, just tension headaches." "I can now tell you what a tension headache is -- and it's nothing. And a sinus infection -- nothing," he said. "It doesn't progress."
Guyuron said most patients, like Calderone, have found that medications don't work or they are unhappy with side effects. 

But Cathy Glaser, a retired lawyer and founder of the Migraine Research Foundation, said she is not convinced surgery is a cure-all for migraines."It's a very complex disease and there are difference kinds of migraines and we don't know the cause," she said. Nerve decompression is only one of about five different surgical techniques from repairing a hole in the heart to implanting electrodes to stimulate the nerves. "Surgical interventions are really an end-of-the-line treatment and not something you are going to do when you are first diagnosed," said Glaser. "It's very extreme and there are side effects." Infection and bleeding can occur, as well as some numbness associated with the surgery that can be long-lasting or permanent. Rarely, minimal hair loss around the incision site can result.
"They go in and open up your head and play with the nerves," said Glaser. "It's a pretty big intervention … You have to be careful when you read the headlines about a cure. But it does help some people." She said studies have been in a population that is small and very selective -- those for whom other therapies have been unsuccessful. Insurance companies are reluctant to cover the procedure while it is still considered somewhat experimental. "The measure of efficacy is different in a lot of studies -- there isn't a gold standard," she said. Causes of migraine can include hormonal fluctuation, particularly in women, and genetic disposition. If one parent has migraine, a child has a 40 percent chance of developing it. With both parents, it rises to 90 percent, according to Glaser. Such was the case with her daughter Samantha, who has had migraines since the age of 3 -- nearly daily since seventh grade.

Samantha Semlitz, 23, of New York City copes with her migraines without surgery. Glaser and her husband began the organization seven years ago when they saw that there was no foundation dedicated to research and a paucity of information. While Samantha was in residential treatment at the Michigan Headache and Neurological Institute, they saw "people of all ages whose lives were destroyed by headaches." Today, Samantha Semlitz is 23 and supports her parents' work. Despite daily pain, she is getting her master's degree in early childhood education at Fordham University and teaches at a school in Harlem. Her headaches build during the day to a crescendo of "throbbing pain" at night. Semlitz is on medication and said she would not consider surgery -- at least not yet. "I have thought about it, but decided against it," she said. "I am not that desperate." "I've had it them so long, I have learned to cope with them," she said. "I haven't known anything different." But she is eager to see more research and helps monitor a Facebook page for the foundation and a group page for those who suffer from migraine. And she continues to work for more research, but understands the challenges in fundraising."It's hard," said Semlitz. "Because people don't die from migraine -- you can't pull out the death card." But she knows how desperate patients are. "You feel completely incapacitated, and feel like you are dying."

Any botulinum toxin administered around the eye has the potential to affect the muscle that opens the upper eyelid. Botulinum toxin can spread from an injection site approximately 1 inch or 2.5 cm. Since botulinum toxin is a paralyzing toxin, the muscle that opens the lid is technically also paralyzed, which may make it difficult to open the eyelid fully.  

CAUSE
Poor location of the injection and excessive doses of botulinum toxin are the culprits of eyelid drooping. Too much botulinum toxin raises the risk of it traveling to areas it shouldn't, increasing the chance of a drooping eyelid. Location of the injection, experience of the person injecting the botulinum toxin and knowing certain facts---such as the patient should remain in an upright position during the injection and three to four hours after injection---are all factors that lessen the risk of drooping eyelid.

PREVALENCE
Drooping of the eyelid happens less than 1 percent of the time. Frown line injections and/or injections underneath the "tail" of the eyebrow (to create a brow lift effect) are the treatments that have the most likelihood of producing a droopy eyelid.
  
DURATION
The drooping eyelid syndrome is not permanent. It will most likely wear off before the botulinum toxin does. Eyedrop treatments, such as Alphagan or Lopidine, can diminish the lid droop by pulling up the eyelid a bit (anywhere from 1 to 3mm). These drops can help stimulate a different muscle that wasn't affected by the botulinum toxin.

Many people are born with “hooded eyes”.  This term can be loosely defined as eyes that are set in such a way that the skin above the eye protrudes over the natural crease of the eyelid.  The crease of the eyelid is the line on the lid that forms the back curve of the eyelid when the eye is open. Hooded generally means eyelids that are partly covered by skin when the eyes are open. The main reason is bone structure, and therefor it can been seen in all ages, even young persons. It’s likely that everyone’s eyes will eventually become hooded as a result of aging. Hooded eyelids tend to drag the eyes downward, sometimes creating a closed and sagging appearance.

Hooded, as it relates to bone structure, describes a brow area above the lids that juts out a bit, causing the skin that covers the area just below the eyebrows to obscure part of the eyelids along the curve of the lid.  In younger people, the skin that covers a portion of the eyelid is not actually sagging, even though it may have the appearance of heaviness. This type of eye structure is also called “bedroom eyes”, as it appears seductive to some people.  Some well known celebrities have hooded eyelids. 
 
The word hooded is also used to describe large eyelids that have no distinct shape or definition.  This type of lid does not have the crease that creates an eyelid in deeper set eyes.
